Most likely, the kitten will need antibiotics from the vet. Being young and skinny, it is also likely that he has worms, which will make it hard for him to put on weight, as well as get proper nutrition. If by outside you mean feral, is it possible to bring him inside, quarantined in a room away from any other pets you have, like a bathroom? This way you can monitor him properly, and make sure he has access to good food. He will need vet attention.

I'm not sure what you mean by "no extra skin" unless it's that you're sure he's not dehydrated. Since you can pick him up, he's not feral which means that when you picked him up, you'd probably get clawed and bitten. There is no "home remedy" for an upper respiratory infection (URI) if that's truly what he has. There are many other diseases he could have that can cause wheezing, etc. If it is a URI, he needs a course of Clavamox or another antibiotic and you'll have to bring him in to make sure he gets his dosages.
